Liability waiver

I agree to the Liability Waiver & Media Release.

Acknowledgment of Risk and Waiver of Liability. I understand that participation in dance classes, rehearsals, and performances with Center Stage Dance Alliance (CSDA) involves physical activity and carries inherent risks, including but not limited to: slips, falls, collisions, abrasions, muscle strains, sprains, broken bones, or other injuries.

By typing my name at registration, I voluntarily assume all risks associated with my own participation and/or my child’s participation in any CSDA class, rehearsal, event, or production.

I hereby release, waive, and hold harmless CSDA, its instructors, staff, officers, and volunteers from any and all claims, demands, or causes of action arising from ordinary negligence resulting in personal injury, illness, or property damage, whether occurring during classes, rehearsals, events, or productions.

Media Release (Photo & Video Consent). I grant CSDA permission to photograph, record, and use images or videos of myself and/or my child in connection with CSDA events, classes, or productions.

Zayne Paresa

Ballet

Zayne Paresa started his performing arts journey with dance classes at Center Stage Dance Alliance. Although his dance education is primarily from Center Stage, he has taken workshops and intensives from renowned dancers in Hilo and Chicago. He has tried all styles of dance, but has a special love for ballet and contemporary. Since starting dance, he has developed a love for all things performing arts and has performed in many local musicals. When he's not on stage, you can find him shooting photos or chilling on the beach.

Before class

What to wear & bring.

Attire Class attire is ballet slippers or pointe shoes, comfortable top or leotard, and comfortable bottoms. You may purchase ballet slippers at the studio or online. Please tie hair back if possible.

Arrival Sign in at the front desk with first and last name. Enter the dance floor with clean shoes or bare feet.

Floor policy Only clean dance shoes or bare feet on the floor. No zippers or metal hardware on clothing.

Bring A filled water bottle.

Observers ‘Ohana welcome to watch — please keep it to 1–2 people per dancer and conversations quiet.

Cancellation policy

Can’t make it to class?

Class credits Fill out the class credit request form and submit before the end of the day of the missed class. Processing fees may apply for refunds.
